यस कम्पनीबाट जारी तपसिलमा उल्लेख भएका बीमालेखका बीमितहरुले बीमालेख हराएको/नासीएकोले प्रतिलिपी बीमालेखको लागि निवेदन पेश गरेको हुँदा उक्त बीमालेख उपर कसैको हकदावी भए यो सूचना प्रकाशित मितिले १५ दिन भित्र कम्पनीको केन्द्रीय कार्यालयमा सबुद प्रमाण सहित हकदावी दर्ता गर्नुहुन सम्बन्धित सबैको जानकारीको लागि यो सूचना प्रकाशित गरिएको छ। सो अवधि भित्र दावी दर्ता नभएमा निवेदकहरुको माग बमोजिम प्रतिलिपी बीमालेख जारी गरिने व्यहोरा समेत सम्बन्धित सबैका लागि जानकारी गराइन्छ।

This notice is published for the information of all concerned that the insured persons of the insurance policies mentioned in the details issued by this company have applied for a duplicate insurance policy due to loss/damage of the original policy. If anyone has a claim on the said insurance policy, they are requested to register their claim with supporting evidence at the company's central office within १५ days from the date of publication of this notice. It is also informed to all concerned that if no claim is registered within the said period, a duplicate insurance policy will be issued as per the request of the applicants.
