हकदाबी सम्बन्धी सूचना
यस कम्पनीबाट जारी तपसिलमा उल्लेख भएको बीमालेखका बीमितले बीमालेख हराएको/नासिएको वा प्रतिलिपि बीमालेखको लागि निवेदन पेश गरेको हुँदा उक्त बीमालेख उपर कसैको हकदाबी भए यो सूचना प्रकाशित मितिले ३५ दिनभित्र कम्पनीको केन्द्रीय कार्यालयमा सबुत प्रमाणसहित हकदाबी दर्ता गर्नुहुन सम्बन्धित सबैको जानकारीको लागि यो सूचना प्रकाशित गरिएको छ । सो अवधिभित्र दाबी दर्ता नभएमा निवेदकको माग बमोजिम प्रतिलिपि बीमालेख जारी गरिने व्यहोरा समेत सम्बन्धित सबैका लागि जानकारी गराइन्छ ।

Notice Regarding Title/Claim
As the insured of the insurance policy mentioned in the schedule issued by this company has submitted an application for a duplicate policy stating that the policy is lost/destroyed, this notice has been published for the information of all concerned to register any claim with evidence at the company's central office within 35 days from the date of publication of this notice if anyone has a claim over the said insurance policy. It is also informed to all concerned that if no claim is registered within that period, a duplicate policy will be issued as per the applicant's request.
