हकदाबी सम्बन्धी सूचना
यस कम्पनीबाट जारी तपसिलमा उल्लेख भएका बीमालेखका बीमितहरुले बीमालेख हराएको/नासिएकोले प्रतिलिपि बीमालेखको लागि निवेदन पेश गरेको हुँदा उक्त बीमालेख उपर कसैको हकदाबी भए यो सूचना प्रकाशित मितिले ३५ दिन भित्र कम्पनीको केन्द्रीय कार्यालयमा सबुद प्रमाण सहित हकदाबी दर्ता गर्नुहुन सम्बन्धीत सबैको जानकारीको लागि यो सूचना प्रकाशित गरिएको छ। सो अबधि भित्र दाबी दर्ता नभएमा निवेदकहरुको माग बमोजिम प्रतिलिपि बीमालेख जारी गरीने ब्यहोरा समेत सम्बन्धित सबैका लागि जानकारी गराइन्छ।

Notice Regarding Claims
Since the insured individuals of the insurance policies mentioned in the details issued by this company have submitted applications for duplicate insurance policies due to the original policies being lost/damaged, this notice is published for the information of all concerned to register any claims on the said insurance policies with supporting evidence at the central office of the company within 35 days from the date of publication of this notice. If no claims are registered within that period, this notice also informs all concerned that duplicate insurance policies will be issued as per the request of the applicants.
