यस कम्पनीले निम्न नाम र ठेगानामा जारी गरिएका बीमालेखहरु हराएको वा आगलागीमा पर्न गएको व्यहोरा खुलाई सम्बन्धित बीमितहरुबाट प्रतिलिपि पाऊँ भनी निवेदन प्राप्त हुन आएको हुँदा उक्त बीमालेखहरुमा हकदाबी भए यो सूचना प्रकाशित भएको मितिले १५ दिनभित्र यस कम्पनीमा उजुरी गर्नुहुन सम्बन्धित सबैको जानकारीका लागि यो सूचना प्रकाशित गरिएको छ । उल्लिखित अवधिभित्र कसैको उजुरी पर्न नआएको खण्डमा निवेदकका नाममा बीमालेख जारी गरिने व्यहोरा यसै सूचनाबाट जानकारी गराइन्छ ।

This company has received applications from the concerned policyholders stating that the insurance policies issued in the names and addresses listed below have been lost or destroyed in a fire, requesting duplicate copies. Therefore, this notice is published for the information of all concerned, requesting them to file a claim with this company within 15 days from the date of publication of this notice if they have any claim on the said insurance policies. It is hereby informed through this notice that if no claim is received within the aforementioned period, duplicate insurance policies will be issued in the name of the applicant.
